Benefits Enquiry Interactive Form

We have an interactive form , for getting benefits advice.
By completing the form you are agreeing to the terms of our privacy policy.

You have the option of using the form in three different ways. You can:

1. complete it on-line, then save a copy  (choose 'File', 'Save as' or 'File', 'Attach to Email' from the Adobe Reader menu) and e-mail it to us at:

   This e-mail address is only for the return of completed forms.  Any enquiries submitted without the form will be ignored.

2. complete it on-line (remember to save a copy for your records - see 1 above), then print it out and send it by regular post*.

3. print a blank copy to complete by hand and send in*.

*Our address is at bottom right of the contact us page.

No. 3. should be your choice if you are using a public computer, to avoid your personal details becoming known to anyone else using the machine after you (just in case it is set up to remember input). 
This does not apply to the other forms on this site, as input to them is not recorded, no matter how the machine is set up.
